The Department of Health and Social Care is reminding people to bring their consent form when getting their Covid-19 Vaccination.
It says consent forms can only be signed by the patient - or in certain circumstances - their GP.
The department is reiterating it cannot be signed be another person on the patients behalf - including close relatives.
